Guidelines

Before completing this online application be sure you have read the Act and Rules and understand the requirements for licensure in Texas.

You may not apply online if:

  1. You are already licensed in Texas and need to renew a current or expired license.
  2. You had a Texas license in the past and want to practice in Texas again.
  3. You have applied for a license within the last 12 months.

You should contact the Board before you apply online if:

  1. You are a "re-examination" applicant, meaning you have previously taken but not passed the national licensure exam.
  2. You are a physical therapy (PT or PTA) applicant who has already registered to take the national licensure exam for the first time in a state other than Texas.
  3. You are an OT applicant from the military and hold no U.S. license.

Information Required to Apply Online:

In order to apply for a license online you must provide the following information. Please have this information on hand, as you cannot save a partially completed online application and return to it later.

  • Name and address information
  • Social security number and driver license information
  • The name, location and school code for the school that granted your degree.
  • Information about licenses held in other states, if applicable.

Completing the Application Process

You have not completed the application process until you meet all requirements for licensure. Other requirements include submission of a photograph of the applicant; a passing score on the online jurisprudence exam; registration for the national exam and/or proof of passing the exam; and several other items, depending on the profession, type of licensure, and method of licensure.
